Sample VOCA RFP Announcement [pdf] and Budget [pdf] – This VOCA Request for Proposals is from New Jersey. Every state’s RFP will be different, but this will give you an idea of what they look like.
VOCA Application Budget Worksheet Instructions [pdf] – This abbreviated version of an application budget shows you how to represent your costs as either requested funds or matching funds.
Sample Organization Budget [xls] – You can add, delete, or change lines in this excel document to match your own organization. This worksheet helps you figure out the budget for your full organization, including your VOCA-funded project and all your other programs combined, and then track the actual income and expenses against your projections to see how you’re doing.
Budget and Matching Funds Worksheet [xls] and Instructions [pdf] – This worksheet lets you track how you are doing each month against your budget and your match. The excel file includes a separate tab for your full budget and another one for your match.
Sample Client Tracking Spreadsheet [xls] – This excel worksheet has built in menus to help you consistently track every service you provide to each of your clients.
Sample Attendance/Timesheet [xls] – As with all our excel files, modify this to fit your organization and then use it to track the work of each of your staff and volunteers.
Sample Confidentiality Agreement [doc] – Your staff, volunteers, and board members will need to keep clients’ information confidential. This document gives you sample language so your organization can adopt a policy that everyone in the organization must follow.
Sample Release Authorization Form [doc] – This is a form for clients to sign that gives your organization permission to share certain pieces of information about them.
Sample Conflict of Interest Policy [doc] – Adopt a policy like this one to stay in compliance with conflict rules and ensure that all staff, volunteers, and board members avoid conflicts of interests.
Sample Equal Employment Opportunity Policy [doc] – Adopt a policy like this one to stay in compliance with your hiring and employment practices.
